Communities of Care is seeking a Home Care Nurse (RN/LPN) to support clients in their homes in the Twin Cities area, including Brooklyn Park. We welcome new grads and provide training; this is a full-time position with a flexible schedule and a caring, client-centered team.

We are hiring a Home Care Nurse (LPN/RN) to join our nursing team to support our clients in their homes. We offer world class home care with small community values. We are looking for Full-time Registered Nurse or Licensed Practical Nurse work with a 61 year-old male in his home. We welcome new grads and will train! We have several clients in the Twin Cities area.

Details for this client's case:

• 61 year old with history of Myotonic Dystrophy Type 1 and restrictive lung disease
• Nurse would also be responsible for attending school with this client.
• Proposed schedule:
• Days: 6:00 AM – 6:00 PM - flexible
• Client lives in Brooklyn Park with his family.

Duties

• Administer medications and treatments as prescribed by the physician

Requirements

Experience:
