Job summary

Communitiesofcaremn is hiring a Home Care Nurse (RN or LPN) to join our nursing team and support clients in their homes in the Twin Cities area. We welcome new grads and will train! This is a full-time position with several clients on our roster.

You will administer medications, monitor patient conditions, assist with personal care, and collaborate with healthcare professionals to implement care plans. You will provide emotional support and education to patients and families, and perform

• RN or LPN Minnesota License is required or currently held.
• Ability to build rapport with patients is essential.

We are looking for Full-time Registered Nurse or Licensed Practical Nurse to work with our client in their homes. We welcome new grads and will train!
