Job summary

Communities of Care in Farmington, MN is seeking a compassionate Home Care Nurse (RN or LPN) to join our nursing team, supporting a 23-year-old female client at home. We welcome new grads and provide training; full-time schedule with mornings/evenings, and some Saturdays as needed.

You will administer medications, monitor vital signs, assist with personal care, document care plans, and collaborate with healthcare professionals to ensure quality, dignified in-home care.

Qualifications

• MN RN or LPN license required.
• Experience in home health preferred.
• Ability to work a schedule including some Saturdays and holidays as needed.

Responsibilities

We are looking for Full-time Registered Nurse or Licensed Practical Nurse work with a 23 year-old female in her home. We welcome new grads and will train! We have several clients in the Twin Cities area.

Details for this client's case:

• 23 year old female with Congenital Myotubular Myopathy,
• Proposed schedule:
• Days: Monday and Fridays 7:00 AM – 7:00 PM - flexible
• Occasional Saturday shifts as needed
• Client lives in Farmington with her family.

Duties

• Administer medications and treatments as prescribed by the physician
• Monitor and assess patient conditions, including vital signs and symptoms
• Assist with personal care activities, such as bathing, grooming, and feeding
• Collaborate with healthcare professionals to develop and implement care plans
• Provide emotional support and education to patients and their families
• Perform various medical procedures based on patient needs
• Maintain accurate and detailed medical records

Requirements
