Job summary

Qualifications

• Administer medications and provide hands-on care with strong clinical judgment
• Collaborate with physicians and providers for timely, coordinated care
• Educate patients and families for informed care planning
• Document accurately in EMR to ensure continuity of care
• Participate in interdisciplinary rounds and care transition processes
• Contribute to a collaborative team environment and continuous improvement
