Job summary

- Coordinate patient care across PACU, inpatient units, Radiology, and anesthesia.
- Develop and implement individualized care plans based on assessments and orders.
- Monitor progress and adjust care plans as needed.
- Collaborate with physicians and staff to ensure coordinated care.
- Educate patients and families about care plans and health management.
- Lead and supervise nursing assistants and support staff.
- Participate in quality improvement initiatives.
