Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide compassionate hospice care during nights, weekends, and on-call hours in Waxahachie, TX. The role emphasizes assessment, care coordination with the IDG, and ongoing plan of care for patients and families.

Responsibilities include direct patient care, charting, and collaboration with physicians. Must hold an RN license and CPR certification; prior hospice or home health experience is preferred.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide high-quality hospice care after-hours, including assessments, visits, and on-call support.
• Coordinate with the IDG to ensure continuity of care between physicians, families, and hospice team.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression and symptom management.
