Job summary

Gentiva, part of Hospice Plus, is seeking an Afterhours RN Care Manager in Rockwall, TX, to provide high-quality hospice care during nights, weekends, and on-call hours. You will assess and coordinate patient and family needs and maintain clinical oversight for assigned patients.

As an active IDG member, you will collaborate with physicians and families, update the plan of care, document meticulously, and support compliance and quality initiatives while upholding compassion and professionalism.

Qualifications

• Active RN license in the state of employment.
• Minimum one year RN experience in acute care, oncology, hospice, or home health.
• CPR certification is required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct in-person care during after-hours, weekends, and on-call.
• Assess physical, emotional, psychosocial, and spiritual needs; develop care plans.
• Coordinate with the IDG and physicians for patient care.
• Maintain accurate clinical documentation and compliance tasks.
• Lead supervision visits and guide field staff as appropriate.
