Job summary
Endeavor Health’s Glenbrook Hospital in Glenview, IL, is seeking an RN Care Manager for weekend nights. This part-time role (20 hours/week) requires IL nursing licensure and a BSN. Expertise in case management and EMR familiarity are highly valued.

Qualifications
- Current professional Nursing Licensure in IL required.
- Bachelor's degree in Nursing from NLN-accredited school preferred.
- Clinical certification such as case management is beneficial.
Responsibilities
- Facilitates payor authorization processes and ensures correct patient type.
- Manages patient care across continuum to remove barriers to timely care and reimbursement.
- Collaborates with CDI team to ensure documentation reflects severity and required services.
- Maintains communication with physicians, nursing and other care-team members for timely progression.
- Identifies and resolves delays to discharge to optimize length of stay.
