Job summary

Endeavor Health is seeking an RN Care Manager for an inpatient role at Glenview, IL. The position is full-time (40 hours/week) with hours Monday–Friday, 8:30 am–5:00 pm and on-call duties including rotating weekends every 3–4 weeks.

Required: IL nursing license and a Bachelor's degree in Nursing; case management certification is a plus, with strong emphasis on utilization management and EMR experience. Collaboration with CDI and multidisciplinary teams is essential.

Qualifications

• Current professional Nursing Licensure in IL required.
• Bachelor's degree in Nursing from a NLN accredited school is preferred.
• Clinical certification such as case management certification is beneficial.
• Basic Life Support for Healthcare providers (AHA) or CPR/AED for Professional Rescuer.

Responsibilities

• Facilitates payor authorization processes and ensures patient type is accurate.
• Facilitates collaborative management of patient care across the continuum to remove barriers to timely care and reimbursement.
• Works with the CDI team to ensure documentation reflects severity of illness and intensity of service.
• Maintains active communication with physicians, nursing and other care team members to affect patient progression.
• Identifies and resolves delays to discharge to optimize length of stay.
• Seeks consultations to expedite care and discharge.
• Monitors progress and ensures the plan of care is patient-focused, high quality, and cost-effective.
