Job summary

Endeavor Health in Chicago, IL is seeking a part-time RN Care Manager to support patients with complex medical and social needs. The role includes conducting thorough assessments, developing care plans with the interdisciplinary team, and coordinating services across transitions of care.

The position requires an Illinois RN license (IDFPR) and a CCM certification is preferred. Weekend 2 shifts of 10 hours each may be included, with premium pay and a strong focus on quality outcomes.

Qualifications

• Bachelors Degree in Nursing required or Masters Degree Nursing preferred.
• 2 years of clinical nursing experience in acute care or hospital setting preferred.
• Current RN license in Illinois (IDFPR).
• CCM certification preferred or CCMC certification in Case Management.
• CPR/BLS for healthcare provider per AHA guidelines.

Responsibilities

• Assess patients' medical, social, and psychological needs.
• Collaborate with providers to build comprehensive care plans.
• Coordinate services and referrals to ensure timely care.
