Job summary

Qualifications

Responsibilities

• Assess, plan, direct and provide quality patient care for assigned patients.

• Coordinate with care team to implement discharge planning and education.
• Maintain accurate records and comply with policies and procedures.
• Engage with physicians, staff, patients and families to ensure safety and care quality.
