Job summary

Forest View Hospital in Grand Rapids, MI is seeking a Psychiatric Triage/Intake Registered Nurse to join our admissions team. The RN will interview patients, document histories, and coordinate transitions to the unit for both adults and adolescents.

This full-time role offers a 36-hour week schedule with 12-hour night shifts (7:00pm–7:30am) and collaboration with ARC staff, physicians, patients and families to ensure a seamless intake process.

Qualifications

Experience in psychiatric nursing or psychiatric assessment is required.
Current CPR or BLS certification is required.
License to practice nursing in Michigan is required or eligible for renewal.

Responsibilities

Interview patients to obtain medical history, medications and health updates.
Document patient history and findings during assessment and admissions.
Assess and reassess psychiatric status and changing needs.
Accompany patient to unit and assist with transition to the unit.
Handle crisis calls, make referrals and complete assessments.
Perform other duties as assigned.
