Job summary

Upperline Health is seeking a weekday Registered Nurse to support a team of clinicians in delivering complex plans of care remotely. This role focuses on telephonic care management and post-discharge coordination to improve patient outcomes under value-based contracts.

Ideal candidates have an Active Compact RN license with at least 3 years of clinical experience; ER/ICU or home health experience is a plus. Weekend/holiday coverage may be required.

Qualifications

• Active Compact RN licensure required.
• Minimum of 3 years clinical experience required.
• Prior healthcare experience serving chronically ill patients preferred.
• Previous experience in ER, Critical Care or Home Health a plus.
• Experience with Athena EMR a plus.

Responsibilities

• Telephonic care management with patients and families, including triaging calls and escalating urgent issues.
• Provide chronic and transitional care management outreach, assessment and intervention.
• Appropriate triage and escalation of routine, urgent and emergent issues.
• Clinical care coordination with other providers and specialists, including clinic, telephonic and home-based visits.
• Medication reconciliation and management, including updates, refills and coordinating with Pharmacy.
