Job summary

Optum Home & Community Care is seeking a National On Call Advanced Practice Clinician for Telehealth Urgent Care. This remote, part-time role provides telephonic/telehealth care to members, with 24/7 coverage including holidays, across markets, using EMR and coordinating with facilities and providers.

You will deliver care, document thoroughly per CMS, participate in care coordination, and meet scheduling commitments while working from a secure location in the U.S.

Qualifications

• Graduate of an accredited MSN or Doctor of Nursing Practice program.
• Active, unrestricted Nurse Practitioner license in CT or RI or ability to obtain.
• Board certified via AANP or ANCC with applicable NP specialty.
• Current DEA licensure or ability to obtain post-hire.

Responsibilities

• Provide telephonic/telehealth care and direction to patients, caregivers and staff for 24/7 coverage.
• Deliver acute and chronic care via telehealth; document per CMS requirements.
• Coordinate care across providers, facilities and care settings; maintain secure remote work.
• Utilize EMR to document encounters and ensure quality elements are addressed.
