Job summary

Upperline Health seeks a weekday Registered Nurse to support clinicians in delivering complex care plans for high‑risk patients under value‑based contracts. The role involves telephonic care management, care navigation, and coordination of post‑discharge services.

Ideal candidates have experience with older adults and telehealth triage, a compassionate and professional demeanor, and at least three years of clinical practice.

Qualifications

• Active Compact RN licensure required (CA, CT, or MI a plus).
• Minimum of 3 years clinical experience.
• Experience with telehealth triage is a plus.
• Experience with Athena EMR is a plus.
• Willingness to provide weekend/holiday coverage.
• Strong writing, organization, and communication skills.

Responsibilities

• Telephonic care management with patients and families.
• Coordinate chronic and transitional care outreach and interventions.
• Triage and escalate urgent patient issues as needed.
• Coordinate with clinics, PCPs, specialists, and home health services.
• Medication reconciliation and patient support coordination.
• Participate in interdisciplinary team meetings and clinical huddles.
• Perform related clinical and administrative tasks.
