Job summary

The role engages with transfer center coordination, supports timely transfers and discharges, and participates in rounds and quality improvement to enhance care progression. BSN is required; MSN preferred, with at least five years of acute care nursing experience.

Qualifications

• BSN from an accredited program; MSN preferred.
• Five years of acute care nursing experience required.
• Two years in emergency nursing, transfer center, or care coordination preferred.

Responsibilities

• Screen patients for anticipated transfers and discharge planning needs.
• Collaborate with the transfer center to arrange transfers between locations and expedite discharge.
• Triage barriers to discharge to optimize hospital capacity and ensure safe transfer and care.
• Maintain effective handoff communication and collaborate with care teams for throughputs.
