Qualifications

• High School diploma or equivalent required.
• Associate's or Bachelor's degree in human service field preferred.
• Two+ years' experience with the population in a patient support role required.
• One+ years' experience as a lead or supervisory role required.

Responsibilities

• Direct, supervise and coordinate patient support team activities.
• Provide medical or therapeutic care under physicians' instructions.
• Coordinate work of staff and set day-to-day performance expectations.
• Guide and direct staff, build trust and cooperation.
• Assist patients with daily living activities and basic health care.
• Lead or co-lead individual or group sessions.
• Oversee admitting and discharging of patients.
• Monitor patients and report changes; maintain progress records.
• Encourage participation in therapeutic and social activities.
• Collaborate with staff to rehabilitate patients and prepare for return to community.
