• Acts as liaison between patient and nurse to report changes.
• Delivers high-level customer service to patients, families, visitors and staff.
• Documents patient care accurately and timely as applicable.

Responsibilities

• Provides basic patient care including care and comfort, vital sign measurement, personal care and hygiene, and assists with mobility, including unit-based duties.
• Acts as liaison between patient and nurse to report changes and/or concerns.
• Delivers high-level customer service to patients, families, visitors and staff.
• Documents patient care accurately and timely.
• Participates in hand-off/report of information upon arrival and departure.
• Performs delegated tasks once competency is validated.
• Mobilizes patients using safe techniques and promotes independence.
