Mount Carmel Health System, a proud member of Trinity Health, is hiring a Patient Care Pharmacist for the Night Shift. You will collaborate with physicians, nurses, and other healthcare professionals to optimize medication use and patient outcomes in a fast-paced hospital setting.

The role requires strong clinical judgment, a Doctor of Pharmacy or bachelor degree, and Ohio licensure, with preference for PGY1 or 3+ years in an advanced clinical setting.

Qualificações

• Bachelor or Doctor of Pharmacy required.
• Current license to practice as a Registered Pharmacist in the State of Ohio.
• PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting is preferred.
• Effective Communication Skills.
• Ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team.
• Performs work independently with minimum supervision, and is effective in a stressful environment.

Responsabilidades

• Coordinates the preparation, distribution, and dispensing of medication in compliance with regulations and policies, directing and assisting pharmacy staff.
• Maintains compliance and regulatory readiness; maintains records of pharmacy transactions for control and accountability of drugs, including controlled substances.
• Assists in managing pharmacy inventory, removing expired or contaminated stock, maintaining perpetual inventories, and processing non-formulary requests.
• Interprets, evaluates and verifies all medication orders.
• Collaborates with multidisciplinary team to optimize drug therapy, adjust dosing, recommend therapies, practice stewardship in antibiotics, monitor for interactions and adverse events.
