Job summary

Homeplace at Midway is seeking a Night Shift Nurse Supervisor to oversee nursing activities on assigned units during night hours. You will complete clinical rounds, address deficiencies, and ensure timely, quality resident care.

You will monitor the 24-Hour Report, coordinate with the Medical Director and on-call staff, review admissions records, maintain accurate documentation, and provide direct care as needed. Strong supervisory skills and a compassionate approach are required.

Qualifications

• Must hold a current RN or LPN license.
• Associate’s degree in Nursing or related field.
• Minimum 2 years’ experience in long-term care with supervisory background.

Responsibilities

• Supervise nursing activities during assigned shifts and address deficiencies.
• Complete daily clinical rounding and identify/rectify performance issues.
• Monitor 24-Hour Report changes and notify Medical Director.
• Review admissions records for smooth transitions of care.
• Respond promptly to resident, family and staff concerns.
• Ensure complete resident documentation (ADL, BIMS, weights, notes).
• Provide direct care as needed and participate in on-call schedule.
• Monitor staffing patterns and adjust assignments.
• Exemplify outstanding customer service to coworkers, clients, vendors.

Job description

Night Shift Nurse Supervisor is responsiblefor the supervision ofnursing activities during assigned duty hours.

Completes clinical rounding for outcomes on your assigned unit(s), daily at a minimum; addresses performance deficiencies as they are identified, with immediate correction.

Tracks the 24 – Hour Report to identify changes in condition, issues that need further investigation, assessment and / or intervention and notifications to Medical Director (MD) and resident representative.

Reviews admissions records for completion of data following the admission process, ensuring smooth transitions of care.

Responds promptly and appropriately to resident, family and staff concerns.

Monitors and ensures completeness of resident documentation including but not limited to; intake, fluid, weights, Brief Interview for Mental Status (BIMS), skin assessments, Activities of Daily Living (ADL) charting and skilled nursing notes.

Provide direct care as needed.

Participates in the On-Call schedule as required and provides direct care

Monitors staffing patterns and addresses with adjustment of assignments.

Personifies outstanding customer service to co-workers, clients and vendors by acting as a subject matter resource in a timely, considerate and friendly manner, always maintaining the “Golden Rule” approach; practices patience and kindness.

Qualifications:

• Required License and/or Certification: Licensed RN OR LPN
• Required Education:Associate’s degree in Nursing or related field from an accredited college / university
• Required Experience:2 years’ experience in long-term care; supervisory experience in acute care, sub-acute or long term care
