Heritage Skilled Care in Carbondale, PA is seeking an experienced RN Supervisor to oversee nursing care on assigned shifts, provide clinical leadership, and ensure regulatory compliance. You will supervise LPNs and CNAs, coordinate with physicians and other departments, mentor staff, maintain clear documentation, and respond to emergencies while upholding high standards of resident care.

• Current RN license in the state of employment.
• Associate or BSN degree; BSN preferred.
• Minimum 2 years RN experience in long-term care or similar setting.
• Supervisory or charge nurse experience required.
• Current CPR/BLS certification (ACLS preferred, depending on facility).
• Strong leadership, problem-solving, and communication skills.
• Proficient in Electronic Health Record (EHR) systems and clinical documentation
• Knowledge of nursing home regulations and resident rights

Responsabilités

• Leadership & Supervision: Act as the nurse-in-charge during assigned shifts, resolving clinical and staffing issues as needed.

• Leadership & Supervision: Assist with new staff orientation and ongoing training and development.
• Leadership & Supervision: Conduct shift hand-off reports and ensure continuity of care.
• Leadership & Supervision: Assist with scheduling and ensure appropriate staffing levels.
• Clinical Oversight: Perform and oversee comprehensive resident assessments and monitor for changes in condition.
• Clinical Oversight: Administer and supervise administration of medications, treatments, and procedures.
• Clinical Oversight: Communicate with physicians, families, and interdisciplinary team members regarding resident care.
• Clinical Oversight: Ensure timely and accurate documentation in medical records.
• Clinical Oversight: Respond to medical emergencies and initiate appropriate interventions.
• Clinical Oversight: Support residents and families in understanding care plans and addressing concerns.
• Regulatory Compliance & Quality Assurance: Ensure nursing practices are in compliance with state and federal regulations, facility policies, and professional standards.
• Regulatory Compliance & Quality Assurance: Participate in audits, surveys, and quality improvement initiatives.
• Regulatory Compliance & Quality Assurance: Maintain infection control and safety protocols throughout the facility.
• Regulatory Compliance & Quality Assurance: Monitor and report incidents or changes in resident conditions per facility policy.

Position Summary:

The RN Supervisor is responsible for overseeing nursing care and staff on assigned shifts to ensure the highest quality of care is delivered to residents. This leadership role involves clinical supervision, direct patient care, coordination with other departments, and maintaining regulatory compliance. The RN Supervisor supports and enforces policies, trains and mentors staff, and serves as a clinical resource within the facility.
