Global Medical Response in Phoenix, AZ is seeking a Nurse Navigator - RN to join our innovative healthcare team. This on-site role focuses on coordinating care and guiding patients through their health journeys using technology, without direct bedside care.

Night shifts are available and the position emphasizes telephonic assessments and compassionate outreach to underserved populations. You will work with a collaborative team dedicated to improving access and outcomes.

Qualifications

• Valid RN license in a compact state.
• 3+ years RN experience in Emergency, Acute Care, or similar fast-paced environment.
• Experience with clinical data collection and patient advocacy.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Bachelor’s or master’s in healthcare administration or public health is a plus.

Responsibilities

• Coordinate care and guide patients through their health journeys without direct bedside care.
• Telephonic patient assessments using evidence-based protocols.
• Connect mobile healthcare resources and integrate services to improve patient outcomes.
• Leverage technology and telemonitoring to track progress and intervene when needed.
• Collaborate with the care team and mentor colleagues on new care delivery approaches.
• Represent GMR and promote our programs within the community.

• Be a Catalyst for Change: Use your nursing expertise to connect mobile healthcare resources, integrating services and innovations to improve patient outcomes.
• Telephonic Patient Assessments: Assess patient needs remotely using evidence-based protocols, helping them navigate the complex healthcare system.
• Empower & Educate: Provide vital health education, support healthy behaviors, and help patients understand their care options across the healthcare continuum.
• Leverage Technology: Engage in telemonitoring activities, utilizing cutting-edge tools to track progress and intervene proactively.
• Lead & Collaborate: Play a key role in team care planning, mentoring colleagues, and championing new approaches in healthcare delivery.
• Promote Our Mission: Represent GMR's innovative programs within the community, reaching populations that need care most but often go underserved.
• Facilitate & Educate: Serve as a facilitator and educator for GMR's integrated healthcare training, empowering others to deliver better care.
• Deliver Excellence: Manage your time, resources, and documentation efficiently while providing outstanding customer service and support.

Minimum Required Qualifications:

• Valid RN Compact State License.
• Minimum of 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment.
• Experience with clinical data collection, ethical principles, and patient advocacy.
• Graduate of an accredited nursing program.
• Current RN license in a compact state.
• Current CPR certification.
• Bachelor's or master's degree in healthcare administration, Public Health, or related fields is a plus, especially if focused on community health or disease education preferred.
