Job summary

Decatur Wellness & Rehabilitation Center is seeking a compassionate LPN for the night shift (6 PM to 6 AM) in Decatur, Tennessee. A $7,500 sign-on bonus is offered. You will lead a care team, administer medications, and participate in admissions, discharges, and transfers while ensuring resident safety and quality care.

Qualifications

• Must hold Tennessee LPN license.
• CPR/BLS certification required.
• Experience in long-term care or skilled nursing facilities preferred.

Responsibilities

• Direct the daily activities of CNAs and ensure adherence to policies and procedures.
• Participate in admissions, discharges, and transfers of residents.
• Assess residents' conditions and administer medications per physician orders.
• Initiate, review, and update care plans.
• Manage nursing care for all residents and delegate tasks to nurses.
• Supervise nurse aides and monitor their assignments.
• Observe residents and respond to changes in condition; inform physicians as needed.
• Maintain confidentiality of protected health information.

LPN Position Summary

As a LPN at Decatur Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Manage the nursing care for all residents and delegate shift tasks to nurses.
• Supervise nurse aides as they complete their assignments.
• Observe residents’ conditions, oversee, and respond as necessary.
• Inform physicians of any changes in resident conditions.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Requirements

• Current unrestricted LPN License for the state of Tennessee
• Current CPR/BLS Certification
