Job summary

Decatur Wellness & Rehabilitation Center is seeking a Night Shift LPN to join our compassionate care team in Decatur, TN. You will direct CNAs, administer medications, participate in admissions and transfers, and ensure high-quality patient care through careful assessment and care planning.

The role requires an active Tennessee LPN license and CPR/BLS certification. The position offers full-time hours with a supportive team and competitive benefits.

Qualifications

• Active LPN license in Tennessee.
• Current CPR/BLS certification.
• Ability to work night shift and supervise CNAs.

Responsibilities

• Direct the daily activities of CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents' conditions, prepare and administer medications per physician orders.
• Initiate, review, and update care plans.
• Manage nursing care for all residents and delegate shift tasks to nurses.
• Supervise nurse aides as they complete their assignments.
• Observe residents’ conditions, oversee, and respond as necessary.
• Inform physicians of any changes in resident conditions.
• Maintain confidentiality of protected health information.

LPN Position Summary

As a LPN at Decatur Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.

• Manage the nursing care for all residents and delegate shift tasks to nurses.

LPN Requirements

• Current unrestricted LPN License for the state of Tennessee
• Current CPR/BLS Certification
