Job summary

Henry County Health and Rehabilitation in Paris, Tennessee, is seeking compassionate LPNs for the night shift (7 PM–7 AM) to join our care team. A $2,500 sign-on bonus is offered. We provide a supportive, team-driven environment focused on compassionate resident care.

You will direct CNAs, participate in admissions/discharges, administer medications per orders, update care plans, and report changes to the nurse supervisor while maintaining patient confidentiality.

Qualifications

• Current LPN license for the state of Tennessee.
• CPR/BLS certification required.

Responsibilities

• Direct the daily activities of CNAs and ensure policies are followed in nursing staff.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents’ conditions, prepare and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report changes in a resident’s condition to the nurse supervisor and inform of factors affecting work performance.
• Perform other duties as assigned by clinical or administrative leadership.
• Maintain confidentiality of protected health information.

LPN Position Summary

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Requirements

• Current unrestricted LPN License for the state of Tennessee
• Current CPR/BLS Certification
