Decatur Wellness & Rehabilitation Center is hiring a Licensed Practical Nurse for the night shift in Decatur, Tennessee. The LPN will supervise CNAs, administer medications, and participate in admissions and transfers to ensure high-quality resident care.

This full-time role requires TN LPN licensure and CPR/BLS certification, with a supportive team and opportunities for career growth.

Qualifications

• CPR/BLS certification required.

Responsibilities

• Direct the daily activities of CNAs and ensure policy compliance.
• Participate in admissions, discharges and transfers of residents.
• Assess residents’ conditions and administer medications per physician orders.
• Initiate, review, and update care plans.
• Manage nursing care and delegate shift tasks to nurses.
• Supervise nurse aides during assignments.
• Observe residents and respond to changes in condition.
• Inform physicians of changes in resident conditions.

LPN Position Summary

As a LPN at Decatur Wellness & Rehab, you’ll be an essential part of our residents’ care. Responsibilities include:

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.

• Participate in admissions, discharges and transfers of residents as required.

• Assess resident’s conditions, prepare, and administer medications per physician orders.

• Initiate, review, and update care plans.

• Manage the nursing care for all residents and delegate shift tasks to nurses.

• Supervise nurse aides as they complete their assignments.

• Observe residents’ conditions, oversee, and respond as necessary.

• Inform physicians of any changes in resident conditions.

• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Requirements

• Current unrestricted LPN License for the state of Tennessee

• Current CPR/BLS Certification
