Job summary

Park Place Transitional Care and Rehabilitation seeks an LPN/LVN to join the unit and support nursing staff in delivering high-quality care. The ideal candidate will supervise nursing assistants, monitor narcotic procedures, and communicate with the Nurse Manager on resident observations and needs.

The role includes documenting patient condition changes, participating in shift reports, and coordinating admissions, discharges, and room changes in accordance with procedures.

Qualifications

• Graduate of an approved school of licensed practical/vocational nursing.
• Current license, in good standing, as an LPN/LVN in the State of Montana or Compact State.
• Devotion to standards and principles of professional nursing.

Responsibilities

• Assist routine daily direction and monitor the work of the nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Communicate on a regular basis with the Nurse Manager or Director of Nursing regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Participate in report at the beginning and end of shifts to assure proper information about residents is communicated.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Assist in the admission, orientation, and observation of new residents, assure that physician orders for diets, therapy, medication, etc. are transcribed and communicated according to procedure.

• $20,000 sign on bonus paid out over a two-year period.
