Job summary

Park Place Transtional Care is hiring LPNs for all shifts in Great Falls, MT. Earn $30 to $38 per hour based on experience, with a $10,000 sign-on bonus. Day shifts are 6a–6p, night shifts 6p–6a, with PRN options.

Full and part-time positions available for compassionate nurses ready to deliver quality care. We welcome multi-state licenses, new graduates, and seasoned nurses. This role ensures quality care through monitoring staff performance, physician contact, care planning, and daily resident

Qualifications

• Graduate of an approved school of licensed practical/vocational nursing.
• Current license, in good standing, as an LPN/LVN in the State or Montana or Compact State.
• Devotion to standards and principles of professional nursing.

Responsibilities

• Assist routine daily direction and monitor the work of the nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Communicate on a regular basis with the Nurse Manager or Director of Nursing regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Participate in report at the beginning and end of shifts to assure proper information about residents is communicated.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Assist in the admission, orientation, and observation of new residents, assure that physician orders for diets, therapy, medication, etc. are transcribed and communicated according to procedure.

This position to ensure that quality care is delivered through monitoring of staff performance, physician contact, care planning and daily resident observation. This position also provides direct care to residents. All this is done according to nursing standards of practice, standard procedures, and established State and Federal regulations for skilled nursing facilities.
