Job summary

Illumination Health + Home in California is seeking a Medical Coordinator (LVN) to deliver high quality medical care to patients experiencing homelessness in recuperative care settings. We have schedules including weekends and nights; this is a part-time per diem position.

Responsibilities include client care, intake, care plans, vital checks, coordination of services, documentation in EHR, compliance with HIPAA, and patient education. The role supports rehabilitation and safe discharge.

Qualifications

• Current and non-restricted LVN license in California.
• 2 years of relevant experience.
• High School Diploma or equivalent.
• Valid CA Driver’s License.

Responsibilities

• Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes.
• Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals.
• Establish and maintain a care plan; set weekly goals for patient to complete and assess medical needs.
• Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing and changes since discharge.
• Coordinate home health visits and referrals; link clients to medical services as needed.
• Promote health literacy and educate patients on illnesses, medications, and prevention.
