Job summary

Illumination Foundation is seeking aLicensed Vocational Nurse (California) to provide care for patients experiencing homelessness in a recuperative setting. The role includes night shifts on weekends, with specific hours 11pm–7:30am, and swing/graveyard differentials as described. You will deliver high‑quality medical care and coordinate services.

Responsibilities include intake, care planning, daily checks, linkage to services, and accurate documentation in the EHR for billing and ongoing care.

Qualifications

• Current and non-restricted LVN license in California.
• At least 2 years of relevant experience.
• High School Diploma or equivalent.
• Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
• Valid CA Driver’s License.

Responsibilities

• Perform initial intake to establish an appropriate care plan.
• Set weekly goals and monitor progress toward medical and social reintegration.
• Conduct morning, afternoon, and evening checks for all clients.
• Link clients to medical services and coordinate home health visits as needed.
• Enter patient information into EHR and prepare reports for billing.
