Job summary

Signature HealthCARE is seeking a dedicated LPN/LVN to join North Hardin Rehab & Wellness Center in Radcliff, KY. This full-time night role (7p-7a) involves caring for residents, supervising aides, and coordinating care with the interdisciplinary team.

You will work 12-hour shifts with weekend rotation, ensuring safety, comfort, and quality in daily activities. An excellent benefits package and sign-on bonus are offered to eligible applicants.

Qualifications

• Current, active LPN/LVN license in KY.
• CPR/BLS certification preferred (AHA or Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR).
• Ability to work night shifts, weekends, and 12-hour shifts.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor and observe residents’ status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate and timely clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and regulatory surveys.
