Job summary

Signature HealthCARE of NORTH HARDIN is actively seeking an LPN/LVN to join our full-time DAY shift team at the North Hardin Rehab & Wellness Center. You will provide direct nursing care, assist with ADLs, and supervise nursing staff to ensure quality resident care.

The role requires an active LPN/LVN license in Kentucky, with a preference for CPR/BLS, and at least 1 year of clinical nursing experience in long-term care. Strong documentation and regulatory knowledge are essential.

Qualifications

Current, active LPN/LVN license in the state of practice.
CPR/BLS certification preferred.
Minimum one year of clinical nursing experience; long-term care experience, preferred.
Experience in a supervisory or charge nurse role, preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.
Effective verbal and written communication skills.
Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
Experience in a supervisory or charge nurse role.
Familiarity with electronic health records (EHR) systems.
Proficiency with Microsoft Office applications.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
Complete accurate and timely clinical documentation per facility policy and procedure.
Coordinate resident admissions, transfers, and discharges according to established procedures.
Participate in quality assurance activities, staff education, and regulatory surveys as needed.
Perform other duties as assigned by the Director of Nursing or designee.
