Job summary

Signature HealthCARE in Danville, KY is seeking a full-time LPN/LVN to provide direct nursing care on the night shift (7:00 PM–7:00 AM). You will administer medications, monitor patient status, and collaborate with an interdisciplinary team.

Requirements include a current Kentucky LPN/LVN license, CPR/BLS certification (preferred), and at least 1 year of clinical nursing experience. Supervisory experience is a plus. We offer a strong benefits package and a $7,500 sign-on bonus.

Qualifications

• LPN/LVN license active in state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum of 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in Supervisory or charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Proficiency with EHR systems and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shift per policies.
• Monitor residents’ status and notify physician/party of changes.
• Develop, implement, and review individualized care plans with the team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities and staff education as needed.
• Perform other duties as assigned by the Director of Nursing.
