Job summary

Appling Healthcare in Baxley, GA is seeking a Licensed Practical Nurse to join our team on the 7P-7A shift. Under the supervision of the Nurse Manager, you will provide direct daily patient care, assess needs, and implement the agreed treatment plan in a long-term care setting.

Required: Georgia LPN license and BLS; 20 hours of education every two years to maintain licensure. Prior long-term care experience is preferred but not required.

Qualifications

• Graduate of an Accredited School of Licensed Practical Nursing.
• Current State of Georgia LPN license.
• Current Basic Life Support (BLS) certification.
• 20 hours of education every two years to maintain LPN license.
• Prior experience in a long-term care facility preferred but not required.

Responsibilities

• Assist with direct daily patient care under Nurse Manager/Charge Nurse supervision.
• Accompany providers with patient rounding and implement doctor’s orders.
• Intervene in patient behaviors using CPI techniques and maintain milieu management.
• Communicate patient updates to families and nursing home staff.
• Participate in treatment team meetings and update care plans.
• Assist with QA, safe medication handling, and unit safety checks.
• Take call rotations and serve as a preceptor for nursing students or new hires.
• Ensure infection control precautions and PPE readiness; document rounds and care plans.
