Qualifications

• Graduate from an accredited practical nursing program.
• Current LPN licensure in good standing.
• BLS certification required by policy.
• Proficiency in English language.

Responsibilities

• Performs nursing assessments and determines patient care needs.
• Recognizes need for immediate intervention when required.
• Documents patient observations, treatments and responses using the nursing process.
• Contributes to plan of care by providing data and suggested goals for RN review.
• Communicates patient status changes to the clinical team.
• Implements the care plan under RN supervision and ensures tasks are completed.
• Monitors patient progress and documents in medical records.
• Organizes workload to deliver quality care efficiently.
• Maintains current nursing practice within legal boundaries and facility policies.
• Documents education and communications to appropriate team members.
