National Church Residences Home Care in Ohio is seeking a Full Time Night LPN to join a team of healthcare professionals dedicated to quality senior care. The role emphasizes supervising nursing care, conducting rounds, and coordinating with physicians and families to ensure consistent, compassionate care for residents.

The LPN will follow established nursing standards, assist with admission procedures, manage medication administration, and supervise nursing assistants.

Responsibilities

• Supervise, provide and direct nursing care per facility policies and procedures.
• Perform daily rounds and develop care plans based on residents' needs.
• Pass medications and treatments following policies; manage MAR/TAR updates.
• Supervise nursing assistants and document performance; coordinate with Unit Manager/Director of Nursing.
• Communicate changes in residents' conditions professionally to residents, families, physicians and staff.
