Qualifications

• Active LPN license required.
• Previous nursing experience; hospice/home-health preferred.
• Strong clinical assessment and patient-care skills.
• Ability to work independently with interdisciplinary collaboration.
• EMR experience preferred.

Responsibilities

• Provide skilled nursing care under RN direction per patient plan.
• Monitor patient conditions and communicate changes to care team.
• Assist with symptom management, meds, and comfort measures.
• Document care and observations in EMR accurately.
• Collaborate with physicians, RNs and caregivers to deliver hospice care.
