Beth Israel Lahey Health is seeking a Per Diem Hospice RN to join the mission‑driven team in Massachusetts. The role focuses on patient/family care planning, symptom management, and care coordination during evening and night hours.

You will access the Hospice EMR, triage calls, visit patients for symptom relief, and ensure consistent high‑quality care across settings. RN pronouncements and death‑related support are part of the duties, with collaboration across MDs, social work, and chaplains.

Qualifications

• Knowledge of pain and symptom management at end-of-life.
• Demonstrated ability to manage all aspects of terminal care/hospice philosophy.
• Able to assess and respond to the needs of patients and families in various settings.
• Ability to provide transportation evenings and nights.
• Ability to manage multiple crisis situations with limited resources.
• Strong organizational and communication skills.
• Ability to function independently and triage care.
• Proficient reading, writing and speaking English.

Responsibilities

• Shares in implementing patient/caregiver plan of care during evening/night hours.
• Accesses Hospice EMR for all pertinent information.
• Triages calls, makes pro-active visits for symptom management.
• Maintains the same level of service during night shifts as during hours.
• Arranges equipment and medication procurement as needed.
• Collaborates with patient’s primary MD and Hospice Medical Director as needed.
• Understands care in SNFs, ALFs and group homes.
• Accesses psychosocial information about home situations impacting care.
• Reports potential problems to Administrator On-Call; coordinates Vigil Volunteer requests.
• Performs RN Pronouncement in all settings except acute care hospitals.
• Provides support to families at time of death.
• Makes pro-active visits to high-risk or new hospice patients in hospitals or SNFs.
• Maintains communication via Hospice EMR (PTCT).
• Provides RN visits during regular hours on a rotating basis to improve continuity.
• Completes Admissions to hospice as needed.
• Provides intake/referral coverage as needed.
• Supervises Hospice Aides in SNFs between 7AM-8AM when assigned.
• Assists in preparing next day visit schedule.
• Performs chart audits, data extraction, medication reconciliation and data input.
• Other duties as assigned by Clinical Director.
