Job summary

VIA Health Partners is seeking a compassionate RN for night shift in Fort Mill, SC, to coordinate patient/caregiver care and perform nursing assessments. You will visit patients, implement care plans, and collaborate with the interdisciplinary team to ensure quality end-of-life care.

We require an RN license (compact where available), CPR certification, and at least 2 years of related experience in hospice or life-limiting illness care. A Diploma/ADN is acceptable; BSN is a plus.

Qualifications

• Diploma or ADN in nursing required; BSN preferred.
• Active RN license; NLC compact license/eligibility.
• Current CPR certification; CHPN preferred.
• Minimum 2 years related experience in life-limiting illness care.

Responsibilities

• Assumes responsibility for coordinating patient/caregiver care.
• Conducts nursing assessments to support patient/caregiver needs.
• Visits to provide appropriate interventions and revises plans of care as needed.
• Develops, implements, and evaluates plans of care including education.
• Emergency triage by telephone and home visits.
• Maintains clinical documentation per guidelines.
