Job summary

VIA Health Partners, Fort Mill, SC, is seeking an After Hours RN for Weekday Nights to coordinate patient and caregiver care and conduct nursing assessments. You will make home visits, follow the plan of care, and adjust it as patient needs evolve.

Requirements include an active RN license (compact/home state), current CPR, and at least 2 years of direct care experience with life-limiting illnesses. CHPN and BSN are preferred.

Qualifications

• Diploma or Associate Degree in nursing required; Bachelor’s Degree preferred.
• Current, active RN license in home state (compact license) required.
• Current CPR certification; CHPN preferred.
• Minimum 2 years of related experience in direct care of life-limiting illnesses.

Responsibilities

• Coordinates patient/caregiver care and conducts nursing assessments.
• Makes visits and follows plan of care; evaluates progress and revises as needed.
• Maintains documentation per VIA Health Partners guidelines and infection control standards.
• Maintains professional licensure and pursues ongoing professional development.
