Job summary

Heartland Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours, offering clinical oversight, assessments, and coordination for patients and families during nights, weekends, and on-call hours.

The role participates in the IDG, manages plans of care with physicians, and ensures compliance, documentation, and timely communication of changes in patient conditions.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments, admissions, routine visits, and live discharges.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Support the IDG process through chart reviews and plan of care updates.
• Educate patients and families on disease progression, symptom management, and hospice philosophy.
• Maintain accurate clinical documentation including assessments, care plans, and notes.
