Job summary

Beth Israel Lahey Health is seeking a Hospice LPN to join Sawtelle House in Reading, MA. The role supports a multidisciplinary hospice team, delivering compassionate nursing care under the supervision of the Hospice RN Case Manager.

Qualifications

• Active Massachusetts LPN license.
• BLS certification required.
• 2+ years as LPN in acute care, rehab, or home care; hospice preferred.

Responsibilities

• Perform nursing visits to patients as assigned by Hospice RN Case Manager/Hospice Clinical Manager.
• Provide hospice nursing care focused on palliative care and symptom management.
• Observe, record, and report changes in patient health to MD and Hospice RN Case Manager.
• Participate in health teaching for patient and family.
• Respect patient/family culture and treatment preferences; communicate with care team.
• Attend regular IDT meetings and maintain HIPAA confidentiality.
