Job summary

Oak Hill in Granby, CT is seeking a Direct Support Professional for the third shift. This full-time role supports individuals with intellectual and developmental disabilities in a residential setting, with a 40-hour week (Sunday-Thursday, 11:00 PM – 7:00 AM).

Compensation is $20.39/hour base rate with differential pay for specific schedules. Due to participant needs, only female applicants will be considered at this time.

Qualifications

• Must be at least 18 years old and have a high school diploma or GED.
• Valid driver's license and ability to transport individuals in agency vehicles.
• Clear background checks, fingerprinting, and health screenings required.
• Complete required Oak Hill training programs and certifications.
• Commitment to a safe, professional work environment and continuity of care.

Responsibilities

• Support daily living activities to promote independence and dignity.
• Monitor health, document information, and assist with medications per plan.
• Assist with mobility, transfers, and nutrition support.
• Transport individuals to appointments and community activities.
• Maintain accurate documentation in the EHR and comply with regulations.
• Respond to emergencies and apply safety procedures as trained.
• Work as a team and participate in trainings and meetings.
