Job summary

Rooted Life is seeking a Community Support Specialist to provide person-centered support during overnight hours, maintaining safety and executing care plans in a supportive housing environment. The role emphasizes welfare checks, documentation in EHR, collaboration with the Care Team, and trainings like Mental Health First Aid and Trauma-Informed Care.

Candidates should have a High School Diploma or GED and be awake overnight with strong observation and communication skills.

Qualifications

• High School Diploma or GED required.
• Experience in residential, care, or advocacy settings preferred.
• Able to stay awake and alert during an overnight shift.
• Strong observation and communication skills with a client-centered approach.
• Proficient with Google Workspace and EHR systems.

Responsibilities

• Conduct welfare checks and monitor client safety during shifts.
• Observe and document changes, escalating concerns to the Care Team.
• Execute care plan activities and assist with system navigation.
• Use motivational interviewing and de-escalation techniques as needed.
• Maintain accurate documentation of encounters and care plan actions.
• Ensure a clean, safe living environment and address health concerns.
• Participate in shift handoffs and communicate updates to the Care Team.
• Uphold confidentiality and professional boundaries at all times.
• Complete required trainings including MHFA and Trauma‑Informed Care.
