Job summary

Dialogue is seeking an Overnight Care Coordinator to ensure smooth continuity of care during off-hours. You will be the first point of contact, supporting members with non-clinical needs, coordinating follow-ups, and ensuring clinical concerns are transferred accurately to the right teams.

Reporting to the Manager, Care Coordination, you will demonstrate empathy, strong communication, and meticulous documentation.

Qualifications

• Proven ability to communicate clearly and empathetically with members.
• Strong autonomy and reliability, able to manage multiple files.
• Commitment to confidentiality and privacy of health information.

Responsibilities

• Respond to member inquiries via chat and phone with professionalism and empathy, while managing expectations regarding timelines.
• Coordinate non-clinical care needs, including navigation and follow-ups, with complete and accurate documentation.
• Identify urgent situations or those requiring clinical escalation and route requests to appropriate teams per protocols.
• Ensure high-quality shift transfers, highlighting priority cases and pending actions to maintain continuity of care.
• Execute operational administrative tasks rigorously, maintaining strict confidentiality of health information.
