Upland Hills Health in Dodgeville, WI is seeking a Certified Nursing Assistant for the Nursing & Rehab Center (Skilled Nursing Facility) on the night shift. This .5–1.0 FTE position includes weekend and holiday rotations, working under the supervision of the nursing team to deliver patient care.

Responsibilities include assisting with daily living activities, direct patient care, and monitoring residents for changes in condition, with emphasis on safety and comfort.

Qualifications

• Must hold a Nursing Assistant certification active in WI and Federal Registries.
• CPR completion preferred; to be obtained upon hire.
• Prior nursing assistant experiences are preferred but not required.
• Outstanding verbal and written communications.
• Prior electronic medical record knowledge is beneficial.

Responsibilities

• Provide general nursing assistant services.
• Assist in direct patient care and observe condition changes.
• Assist with activities of daily living: hygiene, nutrition, mobility, safety.
• Monitor resident status and report treatment adherence issues to the nurse in charge.
• Work under the direction of the nurse in charge.
