Job summary

Transitions Hospice - Illinois is seeking a compassionate Companion Aide CNA for the night shift to provide bedside presence and support to patients in Ottawa and surrounding areas.

You will monitor safety, assist with personal care, and communicate with families while maintaining a calm overnight environment. Current CNA and CPR certifications are required; hospice experience is preferred.

Qualifications

• Current CNA certification meeting state training requirements.
• Current CPR certification.
• 1+ year of hospice, home health, or related healthcare experience preferred.
• Overnight/night shift experience preferred.
• Ability to work independently and remain alert and attentive during overnight hours.
• Strong observational skills with the ability to recognize and report changes in patient condition.

Responsibilities

• Overnight bedside care and safety monitoring.
• Monitor patient condition and report changes to RN on call.
• Assist with personal care including repositioning, toileting, bathing, hygiene, and dressing.
• Provide emotional support to patients and families during the night.
• Ensure proper documentation and adherence to care plans.
