Job summary

Transitions Hospice in Ottawa, IL is seeking a compassionate Companion Aide CNA to join our night shift team from 8 p.m. to 8 a.m. The role focuses on bedside presence, patient safety, and supporting families during the overnight hours.

You will monitor patient condition, assist with personal care, document observations, and coordinate with the on-call nurse to ensure comfort and dignity for terminally ill patients. A CNA certification and CPR are required, with hospice experience preferred.

Qualifications

• Current CNA certification meeting state requirements.
• Current CPR certification.
• 1+ year hospice, home health, or related healthcare experience preferred.
• Prior overnight or night shift patient care preferred.
• Ability to work independently and remain alert during overnight hours.
• Strong observational skills to recognize and report changes in patient condition.
• Genuine compassion for patients and families navigating end-of-life care.

Responsibilities

• Overnight bedside presence and patient supervision to ensure safety and comfort.
• Monitor patient condition, behavior, and environment; report changes to the on-call nurse.
• Prevent falls, elopement, or self-harm through observation within CNA scope.
• Respond to patient requests and maintain a calm, restful overnight environment.
• Assist with personal care including repositioning, toileting, bathing, hygiene, and dressing.
• Observe and document patient condition, sleep patterns, vital signs when ordered; report changes.
