Job summary

Transitions Hospice seeks a dedicated Companion Aide CNA for the night shift to provide continuous bedside presence, safety, and personal care. You will monitor patients, report changes, and support families with calm, compassionate care throughout the night.

Requirements include CNA certification, CPR, and 1+ year hospice/home health experience. Night shift preferred; strong observation and independence a must. Competitive benefits and company support are offered.

Responsibilities

• Document observations and vital signs as directed.
• Support families and maintain confidentiality and compassionate communication.
