Be There When It Matters Most
At Transitions Hospice, we don't just manage care — we protect dignity, comfort, and the moments that matter most. We've reimagined hospice to be something different: compassionate, unhurried, and built around the whole person. That means someone is always present — through the quiet hours of the night — so patients feel safe, families can rest, and no one faces the darkness alone.
If you're a CNA who understands that nighttime presence is its own kind of care, this is your role.
As a Companion Aide CNA on the night shift, you'll be a steady, compassionate presence at the bedside — monitoring patient safety, providing personal care, and giving families the peace of mind that someone is watching over their loved one through the night. Your shift includes:
Overnight Patient Care & Safety
Observation & Documentation
Family Support & Communication
Safety & Equipment
CNAs at Transitions aren't just sitters — they're a vital part of the care team. The overnight hours can be the most vulnerable for patients and families, and our Companion Aides show up with skill, calm, and compassion when it matters most. We give you the support, training, and team to do your best work — even in the quiet of the night.
